Education Grant Request

APPLICATION INSTRUCTIONS

Please submit all of the following documents. If you have trouble downloading the application form, please contact a Baxter BioScience Grants representative at 1-805-372-3315.

  • US Only: Complete and sign the current W-9 Form, with address and information of the Institution/Organization for which payment would be issued.
 
  • Non-US Only: The W-8 Form must be completed ONLY if work is to be performed within U.S. territory. Please complete the appropriate W-8 Form with address and information of the Institution/Organization for which payment would be issued. If all services will be performed outside U.S territory, please state on your institution's letterhead that "No work will be performed on U.S. territory".

  • Include a Letter requesting funding on Institution/Organization letterhead, including educational mission statement, learning objectives, target audience, program outcomes, proposed faculty, and therapeutic areas to be discussed.

  • Include a Preliminary Copy of Program Agenda/Brochure for Funding Requested.

  • Include detailed budget for entire project, and funding amount requested from Baxter. If multiple supporters are involved, please indicate other funding entities.
NOTE:
  • For each faculty member to be paid an honorarium, please include the the hourly rate & total hours worked.

TIMELINES

  • Education Grant Requests less than or equal to $25,000 must be received by Baxter at least ten (10) weeks prior to date of activity.
  • Education Grant Requests greater than $25,000 will be reviewed on a quarterly basis, and must be received by Baxter by the quarterly submission deadline as published on our website. An activity must not be scheduled within ten (10) weeks of the quarterly application deadline. Click here for Quarterly Application Deadlines.

FUNDING GUIDELINES

Education grants approved by the BioScience Review Committee typically range from $1,000 to $25,000, but have exceeded $100,000 for requests demonstrating exceptional educational need, educational design, and compliance to existing regulations and guidances.
